I'm trying to replace the standard bold tag by , with no success.

Is it possible to apply a filter on it?

Not exactly sure what you mean. If you want to create a different button for bold that uses different html you don't replace the existing button. The standard button uses what is provided by the medium editor package. Do not select the "Bold" button from the list of standard buttons for medium editor and then create a "Custom Button" using the controls provided.
