Update examples with newer release version

Most of the examples haven't being updated for a very long time.
Some of them are not running with the latest repo version without minor fixes.

For example:
ocyl/ocyl.rea uses param(33) as user defined parameters, but p33 is occupied by usr default index for bc in v19. Also, most of the map files are not supported by v19.

I think we should create releases of NekExamples for both v17 and v19 to match the corresponding Nek5000 master.
