More information on SmokingModel inputs?

I would like to apply SmokingModel to Optum data for a study (recently read the publication detailing the CROSS model). Where can I learn more about the inputs for SmokingModel, specifically how the cdm database schema and cohort database schema should be structured? Thank you!

Just a heads up that this repo is really old, and code is unlikely to work anymore (especially since in those days we didn't document dependencies.)
