Add newlines in the `focus past list` results for better readability

Currently, the output of focus past list is rendered without spacing between items, which makes it harder to quickly parse past sessions.

Expected behavior:
Each past entry should be separated by a blank line for improved readability.

Example:

Current output (condensed):

2025-09-20 - Session A
2025-09-19 - Session B
2025-09-18 - Session C

Proposed output (with spacing):

2025-09-20 - Session A

2025-09-19 - Session B

2025-09-18 - Session C

Why:

• Improves scanning of history.
• Aligns with typical CLI UX patterns where visual separation helps avoid misreading.

Notes:

• Could be a default change or behind a flag (e.g., --pretty).
