Can't find Personal Teleportation Device

I can't seem to find the personal teleportation device at all. I checked my level 3 forge but I simply do not see it. I also tried going into the config files and changing it to "Inventory" and I still cannot see it. I also tried "MageTable" and same issue. Can you please tell me what I need to do to get this device?
