Keywords in headings are missing a leading space

@torkelrogstad This issue is caused by how titles are handled inside the generated html document. They are decorated with an anchor link id, so they can be referenced from a Table of contents.

Parsed Dom tree example:

The best ways I can think of to resolve this, is to either:

• disable replacements for title sections (line starting with the # character in markdown)
• add a leading space character inside the link, effectively making it [ adress](/_glossary?id=address)
• not advised: mess with the css for a elements to add spacing (this might impact other areas of your site though)

I will try and do both the 'ignoring headers' and 'add an extra space' on my fork of the plugin, and let the user decide by ways of configuration which option to go with.
