Add support for multiple functional and clinical ranges in VA Spec model exports

Although right now we only have one type of clinical classification (pillar_project) and one type of functional classification (investigator_provided) we will likely in the future have various versions of each. We should construct our VA-Spec objects in such a way as to use all available evidence of a type to offer the most complete record of our functional data.
