Define content of readme

• define content of readme:
  • shields (done)
  • IG title, description...?
  • URLs - CI Build - GH + HL7?
• Possibly update the template to generate a fragment of the readme.

The default readme file will contain the badges, and then

• IG title
• IG description
• github build link
• fhir build link
• (canonical link?)
• issues link?
