Deployment guide

It would be good to have detailed guides to the various deployment options, perhaps breaking down into these 3 main sections:

1. shared hosting (list of Camping-friendly options)
2. Linux VPS (i.e. where you have more control than shared)
3. Heroku (the free option)
4. Others?

Many frameworks gloss over this, but (from experience elsewhere) it can present a steep learning curve.

That makes sense to me - I will help with the Heroku section (will need a bit of time though)
