implement health check customization

As a developer/operator I want to be able to be able to customize haproxy health checks.

Examples:

• enable only those Instances that respond in a timely manner to the health check
• enable only those Redis instances that are marked as Role:Master (custom tcp commands).
• enable sophisticated health checks for SMTP, or other applications.
