Remove --exclude-NA option in pVACtools 7.0

This option can lead to confusing behavior, especially with splitting up the percentiles into binding/presentation/immunogenicity. Removing it seems like the best option.

@susannasiebert I don't see this option being passed on the 7.0.0 branch, it looks like it's already been scrubbed, theres no mention of it in argparse lines:

grep -rn 'exclude-NA' .

I do see it on the master branch, and the rst docs have not been scrubbed yet. Can you confirm the branch? Can you also let me know if those .rst files are auto-generated or they need to be altered manually?

It looks like I already made this change as part of #1329 (commit 37d6a2e). That totally slipped my mind. My apologies.
