qt4 is no longer in Ubuntu (cf. https://discourse.ubuntu.com/t/removing-qt-4-from-ubuntu-before-the-20-04-release/12295).
Could this extension be made working with qt5 instead?

by the way, can i create an issue in your fork repository? (settings > features > checklist issues). because there are some that I ask such as:

• is it possible to use the icc address of another folder, in .py you use the fix address /usr/share/color/icc/colord, what if I use from /icc/krita or /icc/scribus ?
• I also recommend using the QDial quality value from 50% to 90%
