Exclude by IP address only does not work

Bug Report

If I configure the Exclude rule with an IP address, and everything else remains as "any" - Stream will continue logging events happing from that IP address.

Expected Behavior

If the IP address is added to exclusion list with any other rules, it should be honored.

Actual Behavior

Stream logs events from the IP address that was added to the ignore list.

Steps to Reproduce the Problem

1. Set Role, Action, Context to Any.
2. Set the IP address to 127.0.0.1
3. Update your profile in wp-admin/profile.php
4. Observe the new record in Stream table of events
