Web interface cannot create DFIQ objects

Web interface encounters an issue in creating DFIQ objects:

Error displayed requires a field to be included but in the web interface, there is no additional fields to match the requirements.
The YAML check is valid.

Hmm what's the URL where this page is loaded? It seems it's not getting the type of the DFIQ object right

The page is directly from the DFIQ. URL endpoint is /dfiq. I tired the same in the /dfiq#scenario url but it returns the same error.
