Added keywords and labels to the excel encounter export. Updated tests.
This builds on PR #1312 which adds the tests.
I have preserved the existing behaviour of the
Reference keywordcolumn and added extra keyword columns to match the behaviour ofEncounterSearchExportMetadataExcel.Fixes issue: #1210
