Notion supports mermaid natively but fenced code blocks weren't mapped. Add direct mapping and test case.

Notion supports mermaid natively, but ```mermaid blocks weren't being mapped correctly.
Solution
Add direct mapping:
map['mermaid'] = 'mermaid'Includes test case. Tested with markdown files syncing to Notion.
